Our TLCEdge Specialty Care Teams consist of a Nurse Practitioner or Physician’s Assistant, a Registered Dietitician (many of whom are also a Certified Diabetes Educator), and a Medical Assistant. The teams work in conjunction with participating Primary Care Physicians, and are led and supervised by Dr. William Fears (Endocrinology) and Dr. Randall Perkins (Bariatrics). Our specific programs include: TLCEdge Chronic Diseases ProgramThere are specific disease states that require specialized support, including Diabetes, Lipid Disorders and Hypertension. We offer information on lifestyle that is specific to the effective self-management of these conditions. TLCEdge Diabetes Self-Management TrainingFor diabetes, our primary focus is on helping the person with diabetes attain the skills and knowledge that translate into a healthier lifestyle – one of living well with diabetes.
TLCEdge Medical Weight Loss ProgramOur TLCEdge Specialty Care Teams conduct a thorough assessment and will work with the patient, the Primary Care Physician, the Bariatric Physician and the Care Coordination Team to support the prescribed weight loss regimen. Our team will work with patients to coach and problem—solve their weight management efforts. TLCEdge Surgical Weight Loss ProgramOur TLCEdge Specialty Care Teams conduct a thorough preoperative assessment and will work closely with the patient, the Primary Care Physician, the Bariatric Surgeon and the Care Coordination Team to reduce risks, to successfully educate the preoperative patient and prepare them for the surgical weight loss process. The team is available to provide comprehensive long term support for the post operative patient. TLCEdge Wellness ProgramThe TLCEdge Specialty Care Teams work in conjunction with the TLCEdge Wellness Program to support behavioral change through personalized web interaction, live seminars and events, teleconferences, and personal coaching.
